			<content:encoded><![CDATA[<p>Predictably but not inevitably the face shows signs of aging. We all see the individual but the artist describes and captures the correct color values, tones and values to fully depict it as it really appears. In this issue, Robert B. Polisky, M.D., board certified in dermatology and internal medicine continues a dialog with two artists regarding the aging face. The first series was developed by Rivka Polisky, a student at Ida Crown Jewish Academy; the 2<sup>nd</sup> professional artist is Jessica Konika. Some of the changes are genetic, some due to sun and others from exposure to cigarette smoke.</p>
<p><strong>“Using artistic principals we can get a better understanding of the science of facial rejuvenation.” – Dr. Polisky</strong></p>

<p>Rivka added changes of skin texture and color and unsightly mottles pigmentation. In addition to degradation of skin, the mass loss is obvious. Lips are smaller and cheek bone and fat pad loss causes a sunken-in appearance. Under eye laxity is depicted as well.</p>
<p><strong>“There is a good deal we can learn about skin rejuvenation from the perspective of the artist.” – Dr. Polisky</strong></p>

<p>Jessica is a professional artist and art teacher. Working with Elk Grove Dermatology, she noted vast changes in skin luster and tone in patients who received the CO2RE laser. She was best able to depict these changes by using classic artistic methods and blending color, shade, tone and shadow.</p>
<p>Dr. Polisky notes that we have a much better understanding of facial flaws and corrective measures if we stand back and consider the face like a portrait. This sidesteps the temptation of zeroing in on one problem area. It also allows us to use perspective and other artistic principals to enhance a person’s appearance by correctly identifying the best cosmetic option whether it is fillers, Botox like products, or CO2RE laser.</p>
<p>If photos alone could depict the aging product and then help us make a final decision, that would be great. The truth is that using older artistic methods and blending them with high tech modalities, the patient is more likely to get an outstanding result and certainly more likely to appreciate the changes.</p>

			<content:encoded><![CDATA[<p>Behavioral researchers are testing online tools to help people get a better grasp of their future. According to the New York Times, by using software, young adults visualize how they will appear and consequently they exercise better judgment in financial decision-making. Our rotating medical student points out that a cool app on the iPad is “Age Booth”. In this app, one inserts today’s photo and sees tomorrow’s version of oneself. In our field of age management, these tools are certainly eye-opening and fun. Whether they are absolutely necessary is debatable. <em>We prefer a realistic view of a patient that</em> <em>addresses primary drivers of the aging process: </em><strong>loss of bony</strong> <strong>mass, loss of subcutaneous fat, weathering and photo</strong> <strong>aging of skin.</strong></p>
<p>Working with each patient is a privilege and a challenge. Dr. Polisky prefers an individualized approach rather than a pat treatment plan. The key comes partly from science but also from <strong>art</strong> <strong>and nature</strong>. We can engineer things to a point that they look unreal and unnatural. It is preferable to work with harmonious forces and turn back the clock in some ways, but always in a balance and with a consideration of each person’s unique facial features.</p>
<p>Getting down to actual cases, the <strong>CO2RE </strong>Laser has proven very valuable. It uses fractional technology allowing customized treatment for different facial zones. <p>Dr. Robert Polisky of Elk Grove Dermatology traveled to St. Louis for a course in “Advanced Facial Surgery and Fat Transfer” earlier this month. Seven surgeons from around the country took the hands-on seminar at the office of Dr. Mike Nayak. <span style="color:#008000;">Here are some observations Dr. Polisky made after attending the two-day course:</span></p>
<p><span style="color:#008000;">1.</span> Downtime is the key concern of patients throughout the country when they investigate cosmetic procedures. Balancing a speedy hassle free recovery with significant improvement is the challenge we discussed at the break sessions at Dr. Nayak’s surgical center.</p>
<p><span style="color:#008000;">2.</span> Global correction of facial aging begins with a carefully performed consultation. This can be further enhanced by good photographic technique.</p>
<p><span style="color:#008000;">3.</span> The latest and most advanced procedures were compared with standard or classic methods. We all had an opportunity to share ideas from our own practice.</p>
<p><span style="color:#008000;">4.</span> Fillers and their optimal use were discussed and demonstrated.</p>
<p><span style="color:#3366ff;">This stimulating session helped us at Elk Grove Dermatology focus on four things we are doing for our patients cosmetic needs:</span></p>
<p><span style="color:#3366ff;">1.</span> Work on a highly significant procedure with 2-7 days of downtime depending on the depth of lasering. The CO2RE™ has shown excellent results and high patient satisfaction in our first year of use.</p>
<p><span style="color:#3366ff;">2.</span> Dr. Polisky’s artistic consultation focuses on balance and correction of complementary facial subunits. In plain English, striving to have different areas of the face in proper proportion with each other.</p>
<p><span style="color:#3366ff;">3.</span> Using his 20 years of experience, Dr. Polisky is comparing standards such as the Obagi Blue Peel®, classic CO2 laser and Smoothbeam™ laser with newer modalities. He is looking for a clear, safe advantage for you.</p>
<p><span style="color:#3366ff;">4.</span> Dr. Polisky pioneered the use of Restylane®, Juvéderm®, Radiesse®, Sculptra® and neuromodulators like Botox®, Dysport® and Xeomin®. Through rigorous study and reevaluation he has come up with some superior injection techniques for customized skin improvement.</p>
<p>The meeting was outstanding! There were plastic surgeons, dermatology fellows and even oculplastic physicians. Still, I left feeling great to be in dermatology. Dermatology brings a unique perspective to the skin and a great understanding not just of the capabilities for improvement, but also the limits of therapy. I hope you will call on us if you need a review of your skins’ appearance.</p>

<p><em>Bicycling Magazine</em> says the most dangerous menace on your ride might be the sun itself. Their favorite sunscreens included Blue Lizard SPF 30 at $12.00 for a 3 oz size and Vanicream SPF 35 sport at $14.00.</p>
<p><em>Consumer Reports</em> chose Coppertone Sweatproof spray SPF 30. Surprisingly CVS and Walgreens brands did just as well as the fancier brands.</p>

			<content:encoded><![CDATA[<p>At 4am the alarm clock sounded, and it was time to get ready to do a live segment on NBC at 6am. I drove down the very quiet roadways and fortunately arrived early enough to find the NBC studios. My plan was to speak about the sun protective clothing from <a href="http://www.sunprecautions.com/">Solumbra</a>, to go over a variety of sunscreens and blocks purchased at Dominick’s, and to point out new features of the skin rejuvenating CO2RE laser. The producer set up a table to showcase the products I brought and after detailing the morning news, the show host, Kim, began the interview.</p>

<p>I started off with sobering statistics:</p>
<ul>
<li>2,000,000 cases of basal or squamous cell carcinomas are present or recur each year. </li>
<li>Alarmingly, melanoma is also on the rise with over 68,000 cases diagnosed each year. </li>
<li>Someone growing up today has a 1 in 5 chance of developing skin cancer. </li>
<li>On a hopeful note, an Australian study of 1,621 patients showed skin cancer risk could be cut more than half by regular application of sunscreen.</li>
</ul>
<p>We discussed UPF (ultraviolet protective factor) clothing.  Using the gloves, shawl, and protective sleeves sent by Shaun Hughes of Solumbra, the point was made that sun protective garb that is tightly woven can be a great starting point for the outdoor person.</p>
<p> Next, the crowded field of sunscreen products was reviewed.  The key buzzword for the FDA is <em>broad spectrum </em>which means UVA (wrinkles, skin cancer) and UVB (sunburn, skin cancer) are both blocked out.  The UVA can find its way right through window glass.  The SPF you are using should be at least 30, and according to the FDA, soon companies will not be able to claim that sunscreens are higher than SPF 50.  Other terms like “waterproof” or “total block” will also be prohibited.  I focused more on vehicles rather than catch phrases, demonstrating the newer wax stick products that are very clean and easy to use, the sprays for larger surface areas, and the places where gels or lotions have a good indication &#8211; such as gels for acne prone skin and lotions for dry skin. </p>
<p>Finally, the idea that nanoparticles could be dangerous was set straight according to current research.  Using titanium dioxide or zinc oxide appears to be a great way of blocking UVA and UVB without the messy look that older blocks caused.  One comment I did not get to make was that most people are ½ to ¼ shy of the 1 oz application (a shot glass) needed to cover their skin with sunscreen.</p>
<p>We next moved to the topic of the CO2RE laser used for the repair of skin damage developed over time as well as by excess sun exposure or smoking.  An interesting picture of a 59-year-old set of identical twins was shown, one of whom was a sun worshipper.  It was easy to see the ravages of sun on the sun loving-twin.  We discussed how the laser, which we have been using in our office, can improve and rejuvenate skin damage.</p>
<p>The 3 minute live segment ended and my time at NBC had come to a close.</p>
